9. Purpose for seeking new evaluation

10. File attachments
All applicants must provide:
  • Technical data that is available, such as Technical Product Literature, Test Results, Acceptance or Evaluations by other organizations in Canada and elsewhere, site monitoring and past experiences.
  • A copy of the installation manual provided to contractors who install the product.
  • A copy of the quality control manual used in the manufacturing plant or the ISO 9000 certificate for the manufacturing plant.
Provide any files required to be included with the application and provide a short description of each. If a file is not available electronically, the information can be forwarded to CCMC by mail or fax.









The maximum size of all files you can attach is 100MB. If form submission is failing, reduce the size of uploaded files or email them to ccmc@ncr-cnrc.gc.ca.
Until a contract is in place, the CCMC quickly reviews file attachments for the information it needs to determine how to proceed. It does not read all of the material, nor carry out an in-depth analysis, until the contract is signed.
11. Sample of the Product

A sample of the product must be provided.

The sample should be a representative portion of the product showing its main components and characteristic elements.
